Parental Consent
This Hold Harmless Agreement (“Agreement”) is entered into by and between _________ (the “Guardian”), on behalf of _________(the “Minor”), and Musical Creations Foundation (the “Foundation”), in connection with the attendance of all Musical Creations Foundation Student events from January 1st 2026 through January 1st 2027.

The Guardian acknowledges and understands that participation in Events may involve certain inherent risks, including, but not limited to, the following:
- Physical injury or harm due to activities at the Event venues.
The Guardian voluntarily chooses to allow the Minor to participate in the Event, fully understanding the risks involved.

The Guardian, on behalf of the Minor, agrees to release, waive, and hold harmless Musical Creations Foundation, its directors, employees, contractors, volunteers, and agents (collectively, the “Released Parties”) from any and all liability, claims, damages, or causes of action arising out of or in connection with the Minor’s participation in the Event. This waiver extends to injuries, damage, or losses arising from:
- Negligence by any of the Released Parties.
- Acts or omissions of third parties, including other event attendees.
- Accidents occurring within the event premises, including but not limited to falls or contact with objects.

In the event of a medical emergency, the Guardian authorizes the Released Parties to seek medical treatment for the Minor, as deemed necessary. The Guardian understands and agrees to assume full financial responsibility for any medical treatment required, including transport, hospital stays, medications, and any other expenses incurred.

The Guardian agrees to indemnify and hold harmless the Released Parties against any claims, damages, liabilities, or expenses (including attorney’s fees) that arise directly or indirectly from the Minor’s participation in the Event or any breach of this Agreement.

This Agreement shall be governed by the laws of the Commonwealth of Virginia, and any disputes arising under this Agreement shall be resolved in the Court of Chancery of Chesterfield, Virginia.
